Improving Health Outcomes: Utilizing Integrated Behavioral Health and Value- Based Payment Modeling to Transform Healthcare with Older Adults

While behavioral and physical health conditions are deeply interconnected, services for medical and behavioral health needs have historically been treated in fragmented, dichotomous systems. Furthermore, the vast majority of healthcare is funded through a payment model based on individual care services provided, without regard to the effectiveness of that care upon payment.  

Integrated Behavioral Health (IBH) is an effort to effectively detect, intervene, and support patient management in behavioral factors related to chronic disease management, mental health issues and substance use, and health behaviors associated with better health outcomes. This is achieved by embedding mental health professionals within the primary care team to partner with medical professionals and offer a team-based, collaborative approach to whole person care.

In today’s discussion, we plan to review how utilizing an integrated approach to primary care funded by Value-based Care, a newer payment model which financially incentivizes effective and quality treatment, can improve health outcomes, patient and staff satisfaction, and reduce overall cost of care in senior health clinics.

Speaker Bios

Jennifer E Hubbard, LCSW, LAC
Jenny Hubbard currently serves as the Lead Behavioral Health Specialist for the Denver market with One Medical Seniors. She earned her Master’s degree focused in Social Work from Colorado State University and is a Licensed Clinical Social Worker and Licensed Addictions Counselor. Jenny has spent the past ten years working in health care and has a passion for improving behavioral health access within the primary care setting, improving health outcomes, and enhancing the partnership between behavioral and medical professionals to better serve patient needs.

Jenny cares deeply about educating and training new health care professionals. She serves as a Field Instructor for graduate social work students and teaches for the University of Denver Graduate School of Social Work as a member of their Adjunct Faculty.

Elle Markman, PsyD, MPH
Elle Markman, PsyD, MPH is the National Director of Integrated Behavioral Health at One Medical, a technology-enabled primary care provider that joined forces with value-based care innovator Iora Health to transform healthcare for patients across the lifespan. She earned her Doctoral degree in Clinical Health Psychology from La Salle University and her Master’s degree in Public Health specializing in healthcare administration, policy and community health from University of Texas Health.

Dr. Markman trained and worked in a variety of integrated primary and specialty medical care settings prior to joining Iora as a Behavioral Health Specialist in 2019. Last year she transitioned to One Medical's Medicare Population Health Program, where she works to drive integrated behavioral health performance and strategic impact on population health, cost of care, quality, and health equity.
