How Community Health Workers fit in Team Base Care

Community Health Workers (CHWs) are non-medical public health workers who connect communities to health care and social service providers.  A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. Bryan Carcamo and Treva Smith will explain how CHWs are essential in helping communities have better health outcomes.  This webinar will also explore how to best utilize CHWs in your organization to improve team-based care.

Speakers

Bryan Carcamo, BS, CHW is a Care Coordinator Supervisor for KC CARE Health Center. He graduated from the University of Missouri–Kansas City with a bachelor's in health sciences. Before becoming a supervisor in his organization, he was a Community Health Worker for two and a half years. Bryan enjoys helping and learning from the community and is a powerful force in the workplace where he uses his positive attitude and energy to encourage others to work hard and succeed. Bryan enjoys learning, researching, and advocating for CHWs in the workplace.  In his off time, Bryan enjoys quality time with family and friends, playing soccer, and traveling.  

Treva Smith, CHW-I is the Workforce Development Specialist for the Community Care Network of Kansas, (the primary care association serving Kansas). Treva is also a Community Health Worker instructor, teaching in both Kansas and Missouri. Treva has dedicated nearly 20 years to studying, educating, and advocating for Community Health. From 2019 to 2024, Treva was an active board member of the Community Health Worker Association of Missouri and is now on the Kansas Community Health Worker Coalition – Education Committee.  In 2022, she was elected by the National Association of Community Health Workers to the board of directors. Treva resides in Topeka, Kansas with her wonderful husband, and has eight beautiful children.
